WHAT DO I NEED TO PROVIDE TO  GET A REPLACEMENT COVID-19 VACCINATION CARD?

To get your order processed, we'll need as much information as possible to ensure that we are able to get your replacement card shipped to you as soon as possible. Please provide all information included in the replacement form. If we have any questions or need any additional information we will contact you via email or by phone. 

DO YOU PROVIDE DIFFERENT SHIPPING OPTIONS? I NEED TO GET MY NEW CARD ASAP!

Yes! For a fee, we offer either 2-day shipping or overnight shipping in addition to standard shipping that should be shipped within 5 business days. Be sure to select the option that is best for you at checkout.

IS MY PERSONAL HEALTH DATA SECURE IF I UPLOAD IT TO YOUR SITE?

Yes! We have partnered with a HIPAA-compliant vendor to ensure you personal healthcare information is safe and secure. Please contact us is you'd like any additional information regarding the processes we have in place.

DO YOU PROVIDE DIGITAL COPIES OF MY COVID-19 VACCINATION RECORD OR JUST PAPER COPIES?

At this time, we only provide paper copies of your replacement COVID-19 Vaccination Record Card. If we can provide you with a digital version of your card we will be sure to update this information.
